Include each resolved claim involving damages resulting from your commercial motor carrier operation. Submissions are confidential and final — please review your entries before submitting.
Step 1 — Identify your company
Provide your USDOT number, an anonymous Respondent ID,
or both — whichever you prefer. The anonymous ID is generated on this page;
only the ID itself is submitted — your name and birth date never
leave this page. Example: John Doe, born April 1977 → JD0477.
Step 2 — Your claims
Enter claims below, or if you already filled out the Excel data template, upload it and we'll fill the form for you. The file is read in your browser and is never uploaded to a server. Every field is optional — provide the information you have and leave the rest blank.
